Q: What's different with custom design and designer wedding dress.

A: Custom design means you get your dream wedding dress made to measure incorporating the features you want.
I'll sketch the design on your silhouette and that's all you get when you place your order.
You'll just have to trust me to create and only get to try it on at your 1st fitting as this beautiful bride did and was thrilled that it's more beautiful than she expected fitting so well except the length which is a smidge long - just slightly higher heels

Designer wedding dress you try samples, order in the closest size and get it altered to fit if needed.

EDIT: The 2nd bride who came in for alterations saw it and fell in love too. The tears and beaming smiles of the entire bridal party makes my day. It was a perfect fit - all that needed done was just shortening the train from royal to cathedral & bustle. WOW WOW WOW I feel like royalty, all other dresses I've tried are now unmemorable, will you name the dress after me? Says the 1st bride who tried this spectacular wedding dress. Yes of course.
