For weddings where I live, in Nova Scotia - in addition to the witnesses, it’s also yours, your fiancés, and both of your parents names and details that also need to be filled out on this form too!! You will likely want to have these details in advance instead of in the moment when all eyes are on you during the signing portion of your ceremony.
Sometimes, if it’s not possible to fill in your parents/witnesses names/addresses in advance or at your wedding ceremony rehearsal the night before for whatever reason, your Justice of The Peace can get this information from you once they arrive on scene to perform your wedding ceremony.
** I will caution that most times in Nova Scotia, the JOP doesnt arrive intil 15-20 minutes prior to your wedding ceremony so this can feel rushy. Check with them to see if they are flexible on this and fill in the boxes the night before, leaving just the official signatures for the day of! 🥰
OR - leave the form blank and fill in and sign everything during your wedding ceremony. You will want atleast 3 songs to play in the background while this is happening to avoid awkward silence. It could take 10-15 minutes.
