REQUIREMENTS:

1. Completed Students Application Form
2. Certified Copy of Latest Grades For Highschool Graduates:/Incoming First Year College
Students: -Certified
Photocopy of Highschool Report Card / DEPED Form 138 For Currently Enrolled College
Students: -Original Copy of Certificate of Latest Semestral Grades Signed by the School
Registrar
3. Certificate of Good Moral Character from the Applicant’s Principal/Dean/school
Registrar/Guidance Counselor
4. Certified Photocopy of Latest Income Tax Return of Parent’s
form BIR, if none, Certificate of Exemption must be Submitted or Affidavit of Non Filing of Income Tax Return
5. Certificate of Indigency/Residency from the Punong Barangay
6. Original Copy of Certificate of Enrollment from Applicant’s School Registrar
7. Two (2) Pieces of Latest 2x2 Photo
8. For Constituents belonging to the Indigenous
People (IP) Community,
a Certificate of Tribal Membership from the Municipal/City Mayor or Recommendation of
the Concerned Municipal/ Barangay Tribal Council Chieftain and NCIP
9. Certified True Copy of Birth Certificate

GUIDELINES:

1. Must be a resident of the Province of La Union whose family gross income does not exceed
P 140,000.00 annually which must be supported by a certified true copy of Income
Tax Return or any legal document equivalent to the same.
2. Must be an incoming or currently enrolled college student.
3. Must be enrolled in any recognized degree courses.
4. Must have a weighted average not lower
than 83% or its letter grade/ numerical equivalent, with no mark lower than 80 %
in any subject except in the case of constituents belonging to the Indigenous People (IP)
Community where a grade of not more than one 78% shall be allowed.
5. Must not be a scholar of other government agencies/offices.
