I finally found it!!! I haven't held a copy of this book in about 50 years. I received an email from Edsy and decided to look for magic props on the site. A book popped up on the search and I looked at the pages. Could this be the book I've been looking for for the past 20 years? It is! Thriftbooks had more of the pages displayed so that I could make a decision about it.
