November 7th is National Retinol Day. Often referred to as the “gold standard” of therapy, the benefits of are well documented in short term studies. But few studies have looked at the efficacy and safety of continued use over a long period of time. So, for this Skincare Monday, we’re highlighting results of a 52-week study that shows the benefits of a stabilized treatment do not plateau over time.

The prevalence of photodamaged skin as well as the development of actinic keratoses and non-melanoma skin cancer is staggering and continues to be a major disease burden on our health care system. About 38% of Caucasians over the age of 50 have at least one in the United States. The high burden of disease costs about 920 million dollars per year and likely this number does not factor in every aspect of the evaluation, treatment, and potential complication of the disease.

During the 2022 conference, Dr. C. William Hanke led a detailed workshop where he provided clarity and therapeutic options for the treatment of in nearly every patient population. Furthermore, his talk delved in the nuances of treatment for superficial type non-melanoma skin cancers. Advisory Council Member and Onsite Correspondent for ODAC shares a detailed summary of Dr. Hanke's workshop. Repost from .

During the 2021 Skin of Color Update, and through concrete examples, Dr. Chesahna Kindred shared some of the intangible benefits of working together to increase patient satisfaction. Hairstylists are key in preventing and helping to treat traction alopecia!

Hairstylists can help to:
*Recommend healthy hair styles
*Train the patient regarding implementing healthy hair styles
*Reinforce your advice

She outlined key important pearls including:
*Stylists are key in prevention (promoting and teaching healthy hairstyles).

*Stylists can triage and recognize alopecia early, so we can do more for the patient.

*As many of these diagnoses are chronic, the stylist can more effectively make sure the patient sticks to their regimen. When you give the patient a counseling form, counsel them to share it with their stylists.

*Stylists can be our partners in education.

*Look for seasoned stylists that are passionate about hair loss while understanding that we each play a special role and should stay within the scope of our training.

This information was presented by Dr. Chesahna Kindred at the 2021 Skin of Color Update conference. The above highlights from her lecture were written and compiled by Dr. Jackie McKesey for Next Steps in Derm.

Atopic dermatitis is a heterogeneous, chronic inflammatory disease that affects upwards of 10% of children and 7% of adults in the United States. Despite being an incredibly common skin concern among patients, AD continues to pose diagnostic and treatment challenges to dermatologists. Repost from .

During the 2022 ODAC meeting in Orlando, Dr. Adam Friedman ( ) did a rapid-fire review of the state of atopic dermatitis with high yield updates in the diagnosis and management of this common inflammatory disease.

Dr. Friedman started with what he has coined the EASY E’s of management of atopic dermatitis: education, expectations, encouragement, and enough:

Education: It is important the patient and anyone involved in their care, including family and teachers, have an understanding of atopic dermatitis. One particularly helpful resource is the National Eczema Foundation that has succinct guides, such as the one found here.

“It’s been a pleasure to work on this novel Atlas project! I have been able to visualize many common and uncommon dermatoses in a wide range of skin tones that I am now able to identify more readily. As a now PGY-2 dermatology resident, it’s helped me tremendously with my clinical gestalt as I see new patients daily. I can only imagine how much this Atlas will positively impact both resident education and practicing dermatologists that may not have much clinical exposure to wide variations of skin types. I’m extremely appreciative of my experience and look forward to more to come in the future.”

At the time of working on the Atlas, Dr. Ezekwe was a Clinical Research Fellow at Henry Ford Hospital System in the Department of Dermatology.

"Through this project, I learned that you can make a positive impact. A group of people can come together, even during a pandemic, to share their talents, knowledge, and skills to bring a vision to fruition. It all started with an idea to improve inclusivity in dermatology textbooks and now is a concrete reality accessible to anyone to improve patient care. It's a process that takes dedication, persistent work, and a passionate drive to have a meaningful influence, and I'm very thankful to have been a part of that process."

“As a future dermatologist, taking part in this initiative has truly been an honor. Not only was I fascinated to learn about the wide spectrum of clinical manifestations of skin diseases in various skin tones, but also inspired by the participants who shared the same excitement as they resonated with the critical need to shift toward more equitable care in dermatology. This is the first of many steps in helping ameliorate disparities that have historically impacted patients and clinicians.”

In the clinical experience of George Han, MD, PhD, treatment of psoriasis currently is often taxing for patients, with wait times to see a dermatologist exceeding 30 days in many markets and patients who present to him having cycled through many providers seeking relief from their disease.

“They come in with bags of topical products to show you what they’ve tried,” Dr. Han, associate professor of dermatology at Hofstra University, Hempstead, N.Y., said during the ODAC Dermatology, Aesthetic & Surgical Conference. “And you’re supposed to see this patient, talk to them, and counsel them in about 10 minutes. How do you make time to conduct an efficient psoriasis visit?” Repost from .

Patients have a long-term battle to get clear, and spending a little longer on the initial visit “pays a lot of dividends,” he said. “Some of these patients are the most thankful patients in our practices, and it truly is gratifying” to see how much they can improve.
